AUTHORIZATION ACCEPTANCE

I authorize the Academy of Social and Applied Science (“ASAS”) to initiate credit entries to the account identified in this form at the financial institution named there (the “Depository”), and, if necessary, to initiate debit entries and adjustments for any credit entries made in error. I authorize the Depository to credit and debit the same to that account.

This authorization remains in full force and effect until ASAS has received written notice from me of its termination, in such time and manner as to afford ASAS and the Depository a reasonable opportunity to act on it.
